HSE prosecutes firms every week. The fines below are criminal penalties, not civil claims, and no insurance policy pays them. For a small firm, the sentencing guideline starting point for the most serious cases is £250,000 (Sentencing Council definitive guideline, micro organisation, high culpability, harm category 1).
