A worker was found collapsed in a pump room after exposure to hydrogen sulphide gas, a by-product of the production process. A delivery driver who went to help was also overcome by the gas. HSE found the company had not adequately assessed and controlled the risk of the gas building up in the site's drainage system and being released from the drains.
