A warehouse supervisor fell 1.5 metres from a mobile loading ramp onto concrete while loading plastic waste bales into a shipping container, after a bale slipped from a forklift and caused him to step backward off the ramp. The manufacturer's instructions required handrails to be fitted before any pedestrian used the ramp, and they had not been.
Injuries: Suffered serious injuries and died in hospital
