Ward Bros (Malton) Ltd fined £250,000 after unsecured metal frames fell back onto a worker during container devanning

What happened

A worker and a colleague were devanning a shipping container in the company's yard, standing up metal frames each weighing approximately 120kg. As the sixth frame was lifted, the fifth frame fell back, followed by the remaining frames, trapping the worker by the neck between the container wall and the fallen frames.

Injuries: Fatal - cardiac arrest; died in hospital three days later.
