A worker reversing a one-tonne dumper truck on a construction site drove it into an unprotected excavation and the dumper overturned. The company had instructed workers not to wear seat belts while operating one-tonne dumper trucks. Operators routinely fastened the seat belt behind them and sat on top of it, which disabled the vehicle's seat belt warning system and gave the false impression that the restraint was being worn. The driver was not wearing a seat belt at the time of the incident.
Injuries: Crushing injuries including a broken foot and ankle.

Never instruct workers to defeat seatbelt safety systems on plant, and protect excavations from vehicle incursion.
