A worker was processing powder fire extinguishers at end of life when a cartridge-type extinguisher experienced a sudden pressure release and exploded, striking him in the head; the extinguisher became embedded in the workroom ceiling. He died on 8 March 2023 at hospital despite medical treatment.
Injuries: Fatal.

Employers must carry out a suitable and sufficient risk assessment, use suitable systems of work for processing end-of-life fire extinguishers, store old extinguishers properly while they wait for processing, and provide a suitable restraint system to contain extinguishers during discharge so dangerous parts cannot be ejected.
