On 18 March 2025 a maintenance engineer was asked to address a conveyor issue during a night shift and, while the machinery was still operating, reached through an unguarded hole to clear a blockage; his arm was pulled into the conveyor. Working alone with no emergency stop button nearby, he had to wave at a CCTV camera for around 40 seconds before he could free himself.
Injuries: Catastrophic injuries to the forearm and hand including a fracture and extensive degloving, requiring multiple operations and blood transfusions; remains unable to return to employment.

“Had the correct protections been in place he would not have been exposed to dangerous parts of a running conveyor through a hole cut into the machine.”
Any access point cut into a machine guard must be properly guarded and an emergency stop kept within reach, and lone workers on dangerous machinery need a way to raise the alarm.
