A breeze block wall that had been backfilled before the mortar had properly set collapsed into an excavation, crushing a steel-fixer against the concrete floor below. The company had no temporary works procedure and had failed to design, coordinate or properly manage the temporary works.
Injuries: Life-changing injuries; hoisted out of the excavation by the fire and rescue service and airlifted to hospital.
