A laminator operative cleaning rollers at the Telford site had a hand drawn into the machine's nip point after a rag became caught during cleaning. There was no fixed guard preventing access to the dangerous moving parts during the cleaning operation.

Give priority to fixed guarding wherever reasonably practicable, and risk assess tasks such as cleaning, maintenance and repair, not just normal production, so the risk of being drawn into a nip point is identified and the safe system gives clear instructions.
