A 20-tonne tipper truck fell approximately two metres off the edge of a bank at an HS2 construction site near Uxbridge and landed on the driver's side. The site's earthworks team had changed their working area that morning, creating a new traffic route but inadvertently leaving an unprotected edge on the bank above. The drivers used the higher-level bank instead of the intended new route; the first vehicle slipped on the ramp and the next driver followed the same path and went over the edge.
Injuries: The driver sustained a broken nose, cut hand and shoulder injury

“Its failure to properly plan and promptly communicate changes in vehicle routes created unsafe conditions for the drivers of tipper trucks.”
Any change to vehicle routes on a live site must be communicated to drivers before it takes effect, not discovered by them on the move.
