A worker climbed inside a conveyor framework to remove plastic packaging from timber pallets. A colleague, unaware of his presence, started the conveyor and crushed him with a three tonne pallet, then reversed it and changed its direction, so the pack moved forward again and caused a second collision. CCTV evidence showed 19 instances of workers entering the danger zone in the weeks before the incident.
Injuries: Fatal; he died at the scene.

Signs and stickers are not enough when workers keep entering a machinery danger zone. Control the risk by physically preventing access (guarding) and by changing the system of work so nobody needs to go inside, rather than relying on instruction alone.
