Multi Packaging Solutions UK Limited fined £433,333 after a suspended 4.5-tonne machine fell and struck a worker's head

What happened

A 4.5-tonne machine was being relocated approximately 160 metres using an improvised system of forklift lifting, skates and manual pulling. The machine had already slid off the forks on at least two occasions before a worker, positioned beneath the suspended load to secure a wheel, was struck on the head when it fell.

Injuries: Severe skull fracture, facial nerve damage, permanent double vision, peripheral vision loss, facial palsy and hearing loss.

Regulation 8(1)(a) and (c) of the Lifting Operations and Lifting Equipment Regulations 1998

The sentence

  • Fine: £433,333
  • Other sentence: £32,500 victim surcharge

Never use an improvised skate-and-forklift system to relocate heavy machinery, and keep workers out from under a suspended load.
