A worker fell through a skylight opening while removing material from a flat roof extension after covers had been removed from two large skylight openings. The company had not properly planned the work at height or put preventative measures such as crash decks or safety netting in place.
Injuries: Serious injuries requiring surgery and long-term treatment.

Any unguarded roof opening must be covered, fenced or otherwise made safe before work near it starts, not left exposed during the job.
