The company built a domestic extension over existing gas pipework and a boiler flue without relocating them, leaving the flue discharging into the new extension. The gas installation was found immediately dangerous and the supply was cut off, leaving the family without gas for weeks.
Injuries: No injuries reported; the family was exposed to risks of gas leaks, fire, explosion and carbon monoxide poisoning.
