A labourer fell through an unprotected stairwell opening while installing wall insulation on the first floor of an apartment block. He had placed a ladder across the opening to reach the top corner.

Work at height must be properly planned and sequenced, with suitable work equipment and measures in place to prevent a fall; a ladder laid across a stairwell opening is not one.
