An employee sweeping debris on the first floor was propelled through an unprotected floor opening onto a concrete surface below when a newly constructed wall collapsed. There was no edge protection, no warning signage, no safety instruction and no supervisor present.
Injuries: Multiple fractures and a dislocated shoulder; one month in hospital.
