An employee was crushed in an industrial cooking machine while attempting to clear a blocked water inlet. The machine's safety systems had been overridden and it was operating while the worker was inside instead of being isolated first.
Injuries: Two broken ribs.

Employers need a risk assessment and a safe system of work for routine tasks like clearing a blocked inlet, and must monitor how the work is really done. Without them, employees devised their own method that overrode the safety systems, when the machine should have been isolated before work began.
