A scaffolder installing temporary edge protection at a site in Keighley stepped on a roof skylight while carrying materials and fell more than 6 metres to the warehouse floor below. STM360 Limited was the principal contractor found in breach of construction regulations over the fragile-surface risk; a second firm, Clover Access Systems Limited, was fined separately in the same case.
Injuries: Broken arm, broken leg and head lacerations.
