An experienced electrician was struck by the blades of a fan while disconnecting a communications cable near it at an underground mine. The fan had been modified in 2020 when a single in-line unit was split into two, leaving the blades just 43mm from the leading edge of the duct, against a guarding standard requiring at least 200mm.
