Site and Field Services Limited fined £40,000 after a worker was entangled in a horizontal borer while cleaning swarf

What happened

An employee was cleaning swarf from a horizontal borer machine when they became entangled in the rotating tool piece, which had remained operational during the cleaning work. This followed previous HSE action at the site over machinery guarding, and a removed trip probe had gone undetected due to failed auditing procedures.

Injuries: Significant leg and ankle injuries, multiple broken ribs, a collapsed lung, skin grafts, and partial amputation of two toes.
