An apprentice joiner was working alone at a residential property undergoing renovation when she was fatally crushed by boards that fell from a stack. The stack, 28 boards of large dimensions, potentially weighing up to 30kg each, had been stacked vertically and unsecured against a wall. The company had no suitable lone working policy or procedure, inadequate site supervision and no suitable policy or procedure for the storage of board materials.

Young or inexperienced workers must never be left alone on site without supervision, and materials must always be stored and secured safely.
